The Special Investigations Unit (SIU), Ontario’s police watchdog, is investigating the tragic death of a 23-year-old woman following a crisis intervention by Durham Regional Police. The incident, which unfolded in Oshawa on the evening of Thursday, April 23, 2026, has left the community searching for answers, especially as it involved a police interaction that led to the woman’s death shortly after being released from a hospital.
The sequence of events began around 9:30 p.m. when officers responded to a call about a woman who was reportedly in crisis in the area of Simcoe Street North and Winchester Road East. Authorities responded quickly, apprehending the woman and transporting her to Lakeridge Health Oshawa hospital for medical attention. It’s understood that she was admitted, evaluated, and then released back into the community shortly after her arrival.
However, the situation took a grim turn when hospital security later observed the woman in the parking garage. This prompted them to notify the police, who returned to the hospital to check on her well-being. Tragically, they discovered the woman lying on the ground below the parking garage, and efforts to revive her were unsuccessful.
